At the onset of evaluating new clients and throughout our tenure acting as care managers, we continually assess the needs of our patients, including the in-home living environment, their medical situation, and any financial concerns.

Safety is our top priority, and we work to ensure our patients have the appropriate supplies and space for movement in their homes depending on their individual needs. We can coordinate the installation of medical equipment and furnishings to help your elderly loved ones comfortably live their life while adjusting to medically-necessary tasks or equipment that may be new to them. On-site visits and communication with in-home care providers also helps us evaluate ongoing needs, such as medication, meals and personal care products.

Our team is also available to provide assistance with financial concerns, including budgeting, paying bills, and getting the appropriate benefits from Medicare, Medicaid, or supplemental insurance.
